Infarto agudo del miocardio
Recomendación T7 – Juicio del Panel y Evidencia
T.7 En personas con infarto agudo al miocardio con supradesnivel del segmento ST (SDST) que no fueron tratados con angioplastia y que hayan completado terapia antiplaquetaria dual (aspirina + clopidogrel) por 3 meses, el Ministerio de Salud SUGIERE mantener monoterapia (aspirina) por sobre mantener terapia dual por 1 año (Recomendación condicional basado en certeza baja de los efectos).
Comentarios del Panel de Expertos:
►La extensión de la terapia dual por sobre este periodo debe considerar las condiciones clínicas del paciente (anatomía coronaria, gravedad de la lesión coronaria, comorbilidades y riesgo de sangrado), las circunstancias sociales (edad, nivel educacional, comorbilidades) y los valores y preferencias de la persona.
►En cualquier caso, la decisión debiera ser tomada en equipo en conjunto con el paciente y el clínico tratante.
El Panel de Expertos analizó y debatió cada uno de las preguntas de la “Tabla de la evidencia a la decisión”, considerando tanto la evidencia de investigación, experiencia clínica, conocimiento de gestión o experiencia de los pacientes. Una vez consensuada la postura del panel respecto a las preguntas, emitieron un juicio seleccionando la opción de respuesta que mejor representaba la opinión del conjunto (destacada con color). Finalmente cuando el panel emitió su juicio sobre todas las preguntas, se emitió la recomendación.
A continuación se presenta la “Tabla de la evidencia a la decisión” con el resumen de los juicios, la evidencia de investigación evaluada, consideraciones adicionales y comentarios planteados por el panel.

a. Se decidió no disminuir por riesgo de sesgo pese a que dos estudios no fueron ciegos [8, 11], ya que el análisis por subgrupo no muestra diferencias entre estudios con bajo y alto riesgo de sesgo.

Evidencia de investigación
No se identificaron estudios que respondieran la pregunta desde la perspectiva de valores y preferencias del paciente.
Sí se identificó una revisión sistemática con metaanálisis que identificó predictores psicosociales asociados a falta de adherencia a medicamentos luego del síndrome coronario agudo.
En el estudio se incluyeron estudios realizados entre 2004 y 2014, de los cuales 17 cumplieron los criterios de inclusión. 8 de 10 estudios encontraron asociación entre depresión y falta de adherencia. Reveló que los pacientes deprimidos tenían el doble de probabilidades de ser no adherentes en comparación con los pacientes sin depresión (OR: 2.00, IC 95% :1.6-3.3, p = 0.015). Se descubrió que la personalidad tipo D predecía la no adherencia en dos estudios en los que se midió (1).
Tres estudios informaron que las creencias sobre el tratamiento basadas en el marco de Necesidad-Preocupación predijeron falta de adherencia a medicamentos y hubo alguna evidencia de que el apoyo social se asoció a mejor adherencia.
No hubo datos suficientes para meta analizar todos los otros factores psicosociales identificados.
Los autores concluyen que hubo alguna evidencia de que los factores psicosociales, en particular la depresión, se asocian a falta de adherencia a la medicación después de un síndrome coronario agudo (1).
Considerar los síntomas depresivos, detectar personalidad tipo D, desafiar las creencias de tratamiento inadaptado y proporcionar un mejor apoyo social para los pacientes pueden ser estrategias útiles para mejorar la adherencia a medicamentos (1).
