En personas con hemofilia con articulación diana y/o sinovitis crónica, el Ministerio de Salud SUGIERE realizar una sinoviortesis radioisotópica por sobre realizar una sinovectomía quirúrgica.
Comentarios del panel:
Si bien la sinoviortesis radioisotópica puede ser una mejor opción terapéutica, en la actualidad, este procedimiento sólo está disponible en algunos centros de salud en nuestro país, dado que requiere de recursos humanos capacitados, infraestructura e insumos específicos poco habituales (como, por ejemplo, el radioisótopo que debe ser importado)
El problema ha sido definido como prioritario en el marco de las Garantías Explícitas en Salud (GES), régimen integral de salud que prioriza un grupo de patologías o problemas de salud, garantizando el acceso a tratamiento oportuno y de calidad.

Moderados: Tomando en cuenta la evidencia identificada, la experiencia clínica y la vivencia de las personas con el problema de salud, el equipo elaborador de la guía estimó que la magnitud de los efectos deseables de «realizar sinoviortesis radioisotópica» en comparación a «realizar sinovectomía quirúrgica» son moderados, y probablemente es importante para la mayoría de las personas.
Consideraciones Adicionales
Es panel destaca que esta intervención no requiere de hospitalización, solo requiere anestesia local, la recuperación es más rápida y no presentan los riesgos de eventos adversos asociados a una cirugía, la rehabilitación y seguimiento es menos intensiva.

Resumen de estudios no comparativos: Si bien los estudios no comparativos no permiten estimar el efecto de una intervención de manera adecuada por las limitaciones propias de su diseño de estudio , se resumen los siguientes resultados:
● Una revisión sistemática [1] que incluyó 18 estudios [13,15-17,19,20,23,24,26,29-32,35-38] (1.880 articulaciones) reportó que la tasa de respuesta (no definida) en pacientes con artritis reumatoide que recibieron sinoviortesis radioisotópica fue de 66,7%.
● Una revisión sistemática [2] que incluyó 9 estudios [11,12,14,17,22,25,27,33,34] (727 articulaciones) reportó que la eficacia clínica observada (criterios de éxito no definidos) en pacientes con artritis reumatoide que recibieron sinoviortesis radioisotópica varió entre 42 a 92% en los estudios prospectivos y entre 61 a 80% en los estudios retrospectivos.
